Nurse Job Description


  • Coordinates services with the Personal Assistant Service Relations Specialist to ensure that identified clients’ needs are met.
  • Assists in development of an individualized plan of care based upon identified Medicaid approved services as determined by BLTC/Medicaid Nurse Reviewer.
  • Assists in modifications and updates to plan of care in accordance to clients’ significant needs/changes per Medicaid guidelines.
  • Participates in weekly New Hire Orientation trainings that are held in the Boise and Caldwell LINC offices.  Trains and signs off on caregiver training matrix to include demonstrating and observing handwashing and transfer skills of new Personal Assistants.
  • Provides RN oversight tasks for Quality Assurance program for 2 LINC offices; Boise and Caldwell.
  • Consistently demonstrates competency with technical nursing skills according to legal scope of practice and within IDAPA regulations.
  • Communicates effectively and in accordance with agency requirements.
  • Documents legibly and according to documentation guidelines and Medicaid standards.
  • Communicates potential risk management issues to administration.
  • Demonstrates teamwork and effective communication to accomplish client, team and agency goals.
  • Communicates potential risk management issues to administration.
  • Timely completion of tasks.
  • Understanding of LINC services in order to provide opportunities for assistance with independent living
  • Participates in staff and other meetings as required or needed to stay current and up to date on company and other information.

*Must have current Idaho RN license.

-Preferably 2 years acute care or home health experience
*Must pass criminal history background check through the Dept. of Health and Welfare.
*Excellent written and communication skills.

*Willing to travel throughout the Treasure Valley and Canyon County Regions.

*Reliable Transportation, personal vehicle will be used for home visits; mileage is reimbursed.
*Knowledge and skills to assist the agency in complying with Medicaid requirements.

*Working knowledge of computers and computer programs as job duties require.

*Flexible Schedule – position will require the RN to schedule home visit appointments that accommodate the Employer’s needs and are expected to be completed in a timely matter as determined by the PAS Director.
